To update the year of diagnosis for your child’s medical records:

1. Go to the Consent Portal and log in via Singpass.
2. Click Medical Information.
3. Under Medical Conditions, select Edit.
4. Select Year of Diagnosis and choose the correct year.
5. Check the box under Acknowledgement.
6. Select Confirm & Submit to save your changes.

You'll see your changes right away on the Medical Information page.
