Who Needs to Attend

As a parent or guardian, you need to attend the appointment.

 

This is so you can agree to any treatments they may need.

 

What to Bring

Bring these documents to register your child:

 

Required documents

Notes

Student card 

 

Health booklet

 

Child’s birth certificate or NRIC

  • We accept original, digital, or photocopies.

Child’s passport or entry permit

  • Only for permanent residents and foreigners.

Previous vaccination certificates

  • Only for vaccination appointments.

 

Note: If you’re a social worker acting as the child’s legal guardian, you need to:

  • Bring an authorisation or court letter to verify your identity
  • Tell the Student Health Centre when you arrive.

 

Additional documents

If you have these documents, bring them too:

  • Referral letter
  • Appointment card
  • Civil service college (CSC) card 
  • Financial assistance documents

 

 

When to Arrive

Come 10 to 15 minutes before your appointment to register.

 

Being on time is important as we’re usually fully booked.

 

If You Are Late

We may not be able to see your child.

If you cannot make it on time, please reschedule your appointment.
