What Is a Health Declaration
The health declaration is a check of your health status. It is based on the Get Active Questionnaire GAQ, which is a self-declared questionnaire that all ages can use.
Note: If you have a health condition, you are encouraged to talk to your doctor before you make a health declaration.
Why Update It
You need a health declaration to join HPB’s physical activities.
If you declare that you’re not ready to participate in physical activities:
- You won’t be able to join HPB’s physical activities.
- You won’t be able to clock MVPA(Moderate to Vigorous Physical Activity) minutes or earn Healthpoints through the MVPA challenge.
This is because your safety is of utmost importance to us.
How To Update
- Open the Healthy 365 app.
- Tap Profile, then Health Declaration.
- Select your current health status and tap Confirm.
When To Update
You need to update your health declaration every year to keep using the Healthy 365 app.
You’ll get reminders if:
- Your health declaration is expiring in 28 days. If you select Skip for Now, the reminder will appear once a day until you update your health declaration.
- Your health declaration has already expired. You’ll be logged out of the app until you update it.
Note:
- If your health status changes, you should update your health declaration.
- You can only update your health declaration once a day. The update will be immediately reflected in your Healthy 365 app.
- You cannot update during a physical or virtual event. Please wait until the event ends.
