About the Health Booklet
When your child is born, the hospital will give you a Health Booklet.
This small book keeps track of your child's health as they grow up. It shows:
- Birth details
- Your child’s growth
- What vaccines they got
- Key health information
When to Bring It
Bring the Health Booklet to:
- School health visits
- Student Health Centre (SHC) appointments
The helps doctors and nurses to check and update your child's records.
Tip: Can’t find your Health Booklet? Bring the medical and vaccination records you have instead.
How to Replace It
You can get a new Health Booklet from:
- SingHealth Polyclinics: $1.50
- Other polyclinics and hospitals: Check with them directly for prices
