About the Health Booklet

When your child is born, the hospital will give you a Health Booklet.

This small book keeps track of your child's health as they grow up. It shows:

  • Birth details
  • Your child’s growth
  • What vaccines they got
  • Key health information

When to Bring It

Bring the Health Booklet to:

  • School health visits
  • Student Health Centre (SHC) appointments

The helps doctors and nurses to check and update your child's records.

Tip: Can’t find your Health Booklet? Bring the medical and vaccination records you have instead.
