Here’s how to update your child’s medical information and consent:  

1. Go to the Consent Portal and select log in with Singpass.

2. Check your details, then select Submit to confirm. 

3. To update your consent, find your child’s name and select Consents. 

4. Check their medical information and select Update Now to confirm. Or select Edit if you need to change it.

 
 

5. Once the details are accurate, select View Consent. 

6. If you need to give a new consent, select Update Now on that consent, then choose Yes. 

7. Choose if your child is left or right-handed. 

8. Check the box to acknowledge the details, then select Confirm & Submit. 

9. If you need to update more consents (such as immunisation, dental, or CVDRF), select Update Now for each one and repeat the steps. 

10. To change the submitted consent, select Edit on the consent. 

11. Check the box to acknowledge the details, then select Confirm & Submit.

Note: If you are sent back to the dashboard after updating your child’s medical information, select View Consent to continue.
