You can choose to:

Details

Stay with the School Health Plan (recommended)

This means that your child can continue to:

  • Receive school health screenings
  • Follow their Lifestyle Prescription
  • Visit their family doctor anytime for additional care.

Opt out completely

This means that your child:

  • Will not receive school health screenings
  • Will not receive a Lifestyle Prescription
  • Will have all their healthcare managed by their family doctor

 

Tip: Whether you stay or opt out, you can always discuss your child's health and lifestyle with your family doctor.

 

Opting Out of the Health Plan

 

You may choose to:
  • Skip the Lifestyle Prescription only
  • Opt out completely
