Steps to Collect a New HPB Fitness Tracker

Select a Collection Method

If you are eligible to collect a new HPB tracker:

 

  1. Open the Healthy 365 app. If you don’t have the latest version, download it from the App Store or Google Play.
  2. Tap HPB fitness tracker on the Home screen.

 

  1. Select Collect a new HPB Fitness Tracker.



  1. Select Self-collect or Appoint someone to collect on my behalf.

Note: Vending machines are for self-collection only.

 

Self-collect from a Vending Machine

  1. Select Vending Machines.

 

  1. Find your preferred vending machine and visit it during the operating hours.   



  1. Prepare your Healthy 365 QR code by either:
    • Tapping Scan QR code on the same screen



    • Tapping Account and then My QR code



  1. Follow the on-screen instructions to scan your QR code.

Self-collect from an HPB Appointed Centre

  1. Select HPB Appointed Centres.

 

  1. Tap Book and manage appointment and then Confirm.



  1. On the BookingSG page, select your preferred location, date and time. You can use Apply Filter to narrow your search by location and date.

 

  1. Enter your details to confirm your booking.  If you want to receive an email confirmation, enter your email address.

 

  1. After your booking is confirmed:
  • You will receive an SMS confirmation.
  • If you provided your email address, you’ll get an email confirmation too.
  • If you don’t see the email, check your junk or spam folder.

On your tracker collection day, bring:

  • Your phone with the Healthy 365 app to show your QR code.
  • Your SMS or email confirmation for the appointment

 

Appoint Someone to Collect on Your Behalf at HPB Appointed Centres

  1. Tap Next then Confirm to go to the BookingSG page.



  1. On the BookingSG page, select your preferred location, date and time. You can use Apply Filter to narrow your search by location and date.




  1. Enter your personal details. If you want to receive an email confirmation, enter your email address.

 

  1. Enter your appointed person’s details. If your appointed person wants to receive an email confirmation, enter their email address.

 

  1. After your booking is confirmed:
  • Both you and your appointed person will receive SMS confirmations.
  • If you provided your email addresses, you’ll get email confirmations too.
  • If you don’t see the email, check your junk or spam folder.
  • Tap the “<” icon to return to the Healthy 365 app.

  1. Tap Share QR code to send it to your appointed person through your preferred messaging app.  QR code will be valid for 30 days from the date you share it.



 

What to bring for the appointment

Your appointed person needs to present:

  • The Healthy 365 QR code you shared
  • The SMS or email confirmation for the appointment
  • Their mobile device with the Healthy 365 app installed (with their own QR code)
