If you are giving consent for the first time, follow this guide to add your child’s medical information and give consent through the HPB Consent Portal.

Who Can Give Consent

You can give consent if you are:

  • The child’s parent or legal guardian
  • Over the age of 21, the legal age for consent

Note: If consent was not given before your child's screening, we will try to contact you for consent.

How to Add Your Child’s Profile

Create your child’s profile in the Consent Portal. You can then manage your child’s medical records and consents online.

To set up your child’s profile:

1. Go to the Consent Portal and select log in with Singpass


2. Check your details and select Submit.


 

 3. Add your child’s personal information.

 

 

 4. Verify your child’s information and select Add Child.

5. Select Okay, Start Now! to add your child’s medical information.

6. Complete the following medical details and select Save and Next after each section:

  • Medical information
  • Allergies
  • Vaccinations

7. Update your child’s vaccination and dental consent, if needed.

8. Choose if your child is left- or right-handed. Select Save and Next.

9. Review all the information and select the acknowledgement.

10. Select Confirm and Submit.

 

Tip: If you have issues logging in with Singpass, you can:

Adding Missing Medical Information

If information is missing:

  1. Select Edit on the medical information page.
  2. Make changes to your child’s details. Select save.

The changes will be updated.

Why Your Information Is Needed

HPB provides health checks, vaccinations, and dental care to keep your child healthy. We need your child’s information to:

  • Give the best care for vaccinations and dental treatments.
  • Spot health issues early to give the right support.

Rest assured that your child’s information is private. It is only used by HPB and authorised agents for your child’s health care.

We need your information to make sure your child gets the right care during health checks.

HPB may:

  • Reach out to you for medical information.
  • Decide on treatment or vaccinations.
