You’ve Lost the Test Guide

Here’s what to do:

You Didn’t Get the Test Kit

Do the following:

  1. Confirm if you submitted the FormSG application.
  2. Share your full address with us.
    • This helps our team check for any mismatch in addresses.
  3. Our team will follow up to track your delivery.

You’ve Lost Your Test Kit

Each participant is only allowed one test kit at each milestone.

If you lost a test kit:

  • A replacement can’t be sent at this stage.
  • You can request a new kit when you reach your next milestone.
  • We’ll send you an SMS with instructions when it is time.

Your Test Kit Is Incomplete or Faulty

To fix this:

  1. Take a clear photo showing the faulty or incomplete test kit.
  2. Email the photo to us at hpb_mailbox@hpb.gov.sg.
  3. Once verified, we will arrange a replacement for you.
  4. If we can’t confirm the issue, it will be classified as a lost kit.

Your Test Result Is Invalid

If the test did not give a proper result:

  1. Take a photo of your invalid test result.
  2. Send us the photo at hpb_mailbox@hpb.gov.sg.
  3. If we can verify it, we’ll send you a new kit.
  4. If it cannot be verified, it will be considered as a lost kit.

You Want To Buy Your Own Test Kit

Cotinine test kits are not available for purchase in Singapore. You can only receive one if you’ve reached an eligible milestone or if your result was verified as invalid.
