Medical Records

Did your child have a health screening at our Student Health Centre?

You can get a printed copy of their records:

How to request

Send an email to Contact_YPS@hpb.gov.sg with:

  • Your name
  • A copy of your NRIC
  • A copy of your child’s Birth Certificate
  • Your address
  • Your phone number

 

Note: If your child is under 21, you need to make the request.

How to collect and pay

You can collect and pay at the same time: 

  • Cost: $60 upon self-collection
  • Where to collect: Student Health Centre Registration Counter, Level 3, Health Promotion Board
  • Payment options:
    • NETS
    • Cash Card
    • Baby Bonus
    • Scan to Pay (such as Nets Pay, DBS PayLah, OCBC Pay Anyone and UOB Mighty)

 

Note: Cash, cheque, credit card, PayNow, and bank transfers are not accepted.

 

Immunisation Records

Check where to find your child’s records based on their birth year:

If your child was born in

1996 or later

Download their records online:

  1. Go to the National Immunisation Registry (NIR).
  2. Select Login with SingPass.
  3. Select the print icon to download your child’s record.

Note: You'll only see records once your child starts primary school. The NIR will keep your child’s records until they turn 18 years old.

Before 1996

Check the printed copies of their:

  • Health Booklet
  • Immunisation Record
  • Birth Certificate

 

X-ray Images

Contact these providers to request your child's X-ray images. Some may charge a fee:
